Guanfacine vs. Clonidine for ADHD: What’s the Difference?

Guanfacine and clonidine are both non-stimulant options for ADHD that work through a different mechanism than either stimulants or medications like Strattera — originally developed as blood pressure medications, later found to help with ADHD symptoms, particularly hyperactivity and impulsivity.

What guanfacine is

Guanfacine (brand name Intuniv in its extended-release ADHD formulation) works on specific receptors in the brain involved in attention and impulse control. It’s often considered when hyperactivity and impulsivity are prominent symptoms, and it can be used alone or alongside a stimulant for added effect.

What clonidine is

Clonidine works through a similar receptor pathway and shares many of the same considerations as guanfacine. It’s also available in an extended-release ADHD-specific formulation (Kapvay), and is sometimes chosen when sedation or help with sleep is also a goal, since it tends to be more sedating than guanfacine for some patients.

  • Sedation level: clonidine is generally more sedating than guanfacine, which can be a benefit or a drawback depending on your situation
  • Timing: some patients take clonidine at night specifically to help with sleep alongside daytime ADHD symptoms
  • Combination use: both are sometimes added to an existing stimulant regimen rather than used alone, particularly when a stimulant helps focus but hyperactivity or impulsivity remain
  • Blood pressure effects: both can lower blood pressure and heart rate, which requires monitoring, especially at the start of treatment or with dose changes

Why these aren’t usually a first choice

Stimulants generally show more robust effects for core ADHD symptoms in most patients, which is why guanfacine and clonidine are more often considered as an addition to an existing plan or an alternative when stimulants aren’t appropriate or well tolerated, rather than a universal first-line choice.

What monitoring looks like

Because both affect blood pressure and heart rate, your provider will check these at the start of treatment and periodically afterward, and will typically want any dose reduction or discontinuation to happen gradually rather than stopping abruptly.

What side effects to expect

Drowsiness, dry mouth, and low blood pressure are the most commonly reported side effects for both medications, typically most noticeable when starting or increasing dose. Most patients find these settle somewhat over the first few weeks, though your provider will want to know if they persist at a level that’s disruptive to your daily life.
